Clearance Forms:

• Child Abuse Form. When filling out the Child Abuse Form, please note the following: 1. Use the office address, so the forms will be sent back to the office. 385 St. Augustine Center, 800 Lancaster Ave. Villanova, PA 19085 2. The County is Delaware 3. Check of the box marked SCHOOL

• Risk and Release Form. Please fill this form out and print back to back so that is one sheet of paper.
• Graterford Prison Clearance Form
