Applications and information for the Spring 2010 Service Break Experiences will be available late-December or early January. Thank you.

Villanova Release and Waiver (to be completed by all participants - mission and habitat)

Photo Release Form (to be completed by all participants - mission and habitat)

Habitat Release and Waiver (to be completed by HABITAT participants)

Motor Vehicle Record Consent Form (to be completed by those driving campus/rental vans)
