PA Professional Personnel ID (PDE #).

If you do not have a PA teaching certification, please type "none" in
I WISH TO ENROLL IN THE FOLLOWING COURSES.
I understand that, by using this online form, I will be charged a $100
deposit for each course
I am requesting. If there is immediate availability in the class, my deposit is non-refundable.
If I am placed on the wait-list, my deposit will
be refunded if I do not eventually gain a place
in the course (this will take place approximately one week after the course begins). If the
course is full, and I choose not to be wait-listed, my deposit will be refunded within one month
of the date of the deposit.
