VTV SERVICE REQUEST FORM

  Please submit a service request at least 10 days prior to your event.
  Any request submitted within 10 days  of the event may be charged
  with a late fee of up to 25% of the final cost of the event. You will
  receive an email confirming VTV service at your event within 3 days.
