Review Process

The annual review is one of the most important things you can do as a supervisor and if you are like most supervisors, probably the most unsettling — both for you and for your employee. There are skills you can learn and tools you can utilize to help you, your employee, your department and the University benefit from the annual employee review process.

To help guide you through the Performance Appraisal process for your employees, we have provided a Tutorial that explains the process, forms, and provides tips for setting goals and conducting effective Annual Performance Reviews.

Forms for the 2009 Appraisal Process

  • Annual Review Form - Annual Staff Employee Review form for use in 2009. Click to open the form, then save it on your local drive. Type in the gray shaded areas and the form will expand to meet your needs.
  • Self Assessment - Individual self assessment form
  • Six Month Review Form - End of Introductory Period/6-Month Performance Evaluation.
