In the space below state, with clarity, all reasons for this appeal. Please type or write all information legibly and be very specific, as the decision of the Appeal Board will be based on the information that you present below. NOTE: A lost permit, forgetfulness, parking for a short duration, failure to properly display permit and/or not seeing posted signs/markings are unacceptable grounds for appeal.
You may attach any pertinent documentation such as diagrams of the manner in which you were parked. This form may be submitted in person, electronically, or by facsimile (610) 519-6563, to the parking office.
Appeal the citation within five (5) working days.
You will be notified via e-mail of the results of your appeal.
