The Application

  1. Please print the Recommendation form (Click here for the recommendation form)  and have a faculty, staff, or other campus leader complete this form following the directions.  They should place the completed recommendation in an envelope and write their name across the sealed back for confidentiality.  They can either send directly or via you to Kim Hill in the Office of Health Promotion on the first floor of the Health Services Building by November 9, 2007.
  2. Please either print the application form (Click here for a Printable Copy)  and submit to Kim Hill in the Office of Health Promotion on the first floor of the Health Services Building or complete the online version below by November 9, 2007.
